Curing disease, or any other illness, is between you and your health care/medical professional.  Twila Johnson, nor any associates of Nurture by Nature, do not treat any diseases or illness nor make any diagnosis of any illness.  Further, Twila Johnson and any associates of Nurture by Nature are not medical doctors and are not attempting to portray themselves as, or conduct the activities, of medical doctors.

 INFORMED CONSENT

Neither Twila Johnson or any associates related to Nurture by Nature do any of the following things, either implied or intended:

1.      We do not diagnose.

2.      We make no attempt to cure any condition.

3.      We make no claims or imply any claims that suggestions are given to cure any condition.

4.      We do not claim that any supplemental material we may speak about will cure any condition, or that its' purpose is to treat any condition.

5.      We do not prescribe or treat disease, however, we do attempt to educate you in/on dietary recommendations and exercise if it is not contradictory to the recommendations of your primary physician.

I, the undersigned client, understand the above statements. I, as the client, understand that diet and nutrition is considered to be an inexact science and that the results obtained are not always constant or predictable. I also understand that there is no guarantee of any results and the opposite of the desired results may appear. Whether or not I participate in this procedure or program is my decision, based on my constitutional right of the Ninth Amendment. All decisions relative to my well being and health must be made by me. I further understand that Twila or the staff of Nurture by Nature are not medical doctors and are not attempting to portray themselves as, or conduct the activities of medical doctors.  If any representations have been made to me concerning this program or if I have any understanding about this program which representations and/or understandings are contrary to any of the above statements. I will indicate so at the bottom or reverse side of this form.

 ARTICLE IX, U.S. CONSTITUTION

“The enumeration in the Constitution, of certain rights, shall not be construed to deny or disparage others retained by the People”

Under the Ninth Amendment to the Constitution of the United States of America, I retain the right to freedom of choice in health choices.  This includes the right to choose my diet, and to obtain, purchase and use any therapy regimen, modality, remedy or product recommended by my therapist, doctor or any practitioner of my choice.

The enumeration in this declaration of these rights shall not be construed to deny or disparage other rights retained by me, or my right to amend this declaration at any time.

CONSTRUCTIVE NOITCE

Notice is hereby given to any person who receives a copy if this Declaration and who, acting under the color of the law, intentionally interferes with the free exercise of the rights retained by me under the Ninth Amendment, as enumerated in this declaration, that they may be in violation of my civil and constitutional rights, Title 42, U.S.C. 1983 et seq. and Title 18, section 241.
